Registration for a minor requires the Registration Form,
the Liability Release Form - Child, and the Parental Authorization for Treatment.
Registration for an adult requires the Registration Form,
and the Liability Release Form - Adult.
The Credit Card Authorization Form is optional. Please complete if you
would like your credit card to be charged monthly for your tuition payment.
