Registration for a minor for a Summer Program or 8-week session requires the
Special Session Registration Form,
the Liability Release Form - Child,
and the Parental Authorization for Treatment.

Registration for a minor for the Fall Session requires the
Registration Form,
the Liability Release Form - Child,
and the Parental Authorization for Treatment.

Registration for an adult requires the
Special Session Registration Form,
and the Liability Release Form - Adult.

The Credit Card Authorization Form is optional.
Please complete this form if you would like your credit card
to be charged monthly for your tuition payment.
